Chronic diseases are the leading cause of death, disability and healthcare spend in the U.S. and nearly three-fourths of adult hospitalized patients have multiple chronic conditions and multiple sources of care. Unfortunately, once these patients leave the hospital, their providers do not have a good way to coordinate with one another, leading to collaboration failures and negative health outcomes. With U.S. healthcare ranking last in the developed world despite incomprehensible spending, we knew there had to be a better way. We founded Watershed in 2013 in order to solve this critical problem.
We studied the benefits of electronic health records, and dug into the challenges that were causing health information exchanges to fail. We worked with hundreds of providers across the care continuum and built a solution that has been proven effective, reducing 30-day hospital readmission rates by over 30% for Medicare patients across 5 hospitals over a 5 year period.* Now, our mission is to bring Watershed to patients across the country, improving health outcomes for all.
